Probably not a dealbreaker if everything else is okay. If things are not okay, or if it is just that important to a person...then it could be the straw that breaks the camel's back.

I say no deal breaker. People have their quirks, that one seems fairly harmless. As long as other signs of affection are present, I'd chalk it up to no big deal.
Personally, I think it can be a symptom of a bigger intimacy issue in general, though it depends on, like you said, Dax, whether one is affectionate in other ways.
There's a difference between being anti-handholding and not into holding hands.
Fortunately, the issue seems to have resolved itself a bit for my friend's friend.
